  • How It All Started – Meet Your Dentist in Ruabon

    By Dr John Barclay | DRJB Smile Clinic, Ruabon, North Wales 3D Scan | Whitening | Composite Bonding | Veneers | Invisalign 👶 It Started in 1984 — In Our House When people ask me how long our clinic’s been running, I say: “Since 1984… technically from our living room.” My Mum and Dad built the original surgery from scratch — not in a rented unit or business park, but inside our own house. The study was the surgery, the living room became the waiting room, the kitchen was the lab, the decon room, the stock cupboard, and the tea station. Mum was the receptionist, hygienist, decorator, and accountant (dentistry wasn’t quite as regulated back then). Dad was the dentist, plumber, fixer, and everything else. And me? I was the baby in the waiting room — literally. Sat in my high chair, raking in 50ps from patients like some miniature receptionist with charm. (Adjusted for inflation, I’m owed a small fortune.) The chair marks are still engraved in the parquet floor. And to this day, the smell of developing fluid still takes me straight back to that kitchen–slash–lab–slash–brew station. 🛠️ Growing Up in a Dental Clinic (and Sleeping in the Waiting Room) After 18 months, the surgery moved next door into the old doctor’s surgery — which meant we got our house back. Sort of. Since I was a baby, our weekends and holidays weren’t for lie-ins or beach trips. Me and my little brother were: Painting surgery walls Tiling floors Carpeting cupboards Digging up concrete Fixing suction units And once — lugging a dental chair up a flight of stairs and around two tight corners (never again). I didn’t want to be a dentist back then. I wanted to be a volcanologist and chase lava around the world. How it all started - 1984 (According to AI) However, when university forms arrived, something drew me back. Dentistry was suddenly on the list again. So I tested it the best way I could: I spent my 6-week summer holidays nursing alongside Dad, who (very cleverly) tried to put me off by throwing me into blood, vomit, and hard work. It backfired. I absolutely loved it. 🧬 The Long Way Round I didn’t get into dental school the first time — because I missed the final page of my A-Level Biology paper (yep… 20 marks gone). So I did a degree in Physiology first, then got into dental school. Honestly? No regrets. I got 8 years of uni, played hockey, made lifelong friends, and discovered that the human body is amazing. I even did a year of MaxFax (maxillofacial surgery) — and that was an education in itself. Happy not to see dog bites on the neck again, or sleep on the floor waiting for the bleep to go off at 3 am, or stitch drunks back together in the wee hours… But I learned a lot. And I still use that knowledge to this day — especially when understanding how dentistry fits into the bigger picture of health, trauma, and recovery. It also reminded me that I really, really liked teeth. And not working through the night! 🧑‍⚕️ Full Circle: Returning to Ruabon in 2013 I did my foundation year in 2011 with Dad — because I knew I could ask any question and never feel stupid. In 2013, I came back for good. By 2018, Mum and Dad retired. Mum skipped off into the horizon, while Dad did a 3-day week, then a 1-day week, and now finished for good! Everyone shed a tear or two, or three. (Although if you know my dad, you’ll know he’s busier than ever — not quite sure how they ever ran a business on top of that.) Now I’m the one: Treating patients who used to hand me 50ps Running the practice they built And getting babysitting flashbacks from Andrea and Helen, who looked after me and my brother when we were toddlers. (Yes, I still remind them — even now I’m over 40.) 🔧 What’s Changed (and What Hasn’t) Dentistry’s moved on. Long enough that some of the patients who sit in the chair remember my dad sitting in it first — on the other side of it, looking at their teeth, knowing their names, knowing their children's names, knowing who'd had a hard year and who hadn't. He built this from nothing. Seven days a week for years, alongside my mum, to give his children options. He didn't build a business. He built a community of patients who trusted him enough to keep coming back for decades. When he retired, he handed me his patients. Not a list. Not a file. People. People who had known him for twenty, thirty years. That isn't something you inherit lightly. What He Taught Me That Isn't in a Textbook He mentored me through five years after foundation training. Gradually letting go of the reins. Trusting me with more, then more, then everything. The clinical skills were part of it. The deeper lesson was something else. He treated patients like family. Not as a marketing line — as a fact. He knew things about their lives that had nothing to do with their teeth. He remembered. He asked. He cared whether they were alright, not just whether their occlusion was. I watched him do this for years before I understood what I was watching. It wasn't a bedside manner. It was a philosophy.   • When Periodontal Disease Complicates Endodontic Treatment.

    By Dr John Barclay | GDC No. 210844 | DRJB Smile Clinic, Ruabon, North Wales The endo-perio lesion is one of the most diagnostically challenging presentations in general dental practice. The clinical picture is often ambiguous, the treatment sequence is counterintuitive to GDPs who haven't seen many of them, and the prognosis depends almost entirely on getting the diagnosis right before any treatment begins. Here's the framework I use. The Classification That Matters The traditional classification divides endo-perio lesions into three groups: primary endodontic with secondary periodontal involvement, primary periodontal with secondary endodontic involvement, and true combined lesions. That taxonomy is clinically useful because it determines treatment sequence — and getting the sequence wrong is the most common mistake. Primary endo with secondary perio: The tooth has pulpal necrosis. Bacteria from the infected canal have tracked along the root surface, triggering a pocket that looks periodontal but isn't. Vitality testing is negative. The periodontal involvement is a consequence of the endo pathology, not an independent disease. Treat the endo. The perio resolves. No perio treatment required. Primary perio with secondary endo: Long-standing marginal periodontitis has tracked apically along the root surface, eventually compromising the pulp through accessory canals or the apical foramen. The tooth may still test vital — weakly, or inconsistently. The perio is the primary disease. Endodontic treatment may be needed, but treating the endo alone will not resolve the perio. These cases carry a significantly worse prognosis than primary endo lesions. True combined lesion: Independent endo and perio pathology that have merged into a single defect. The worst prognosis of the three. Treat both. Manage expectations carefully. Diagnosis Before Treatment The single most important step — and the one most often skipped in a busy general practice — is determining which type of lesion you're dealing with before committing to a treatment plan. Vitality testing is essential. A necrotic tooth with a deep narrow pocket tracking to the apex is almost certainly a primary endo lesion. The same pocket on a tooth with a positive vitality response is almost certainly primary perio. These are different cases with different treatments and different outcomes — but they can look identical on a periapical. CBCT changes the picture significantly. A two-dimensional periapical will show a periapical lesion and a bony defect. A CBCT shows the three-dimensional extent of that defect — whether it's circumferential, whether it's tracking along one root surface, whether there's furcation involvement. That information determines both the diagnosis and the prognosis before a single instrument touches the canal. Probing is also critical. A narrow, deep probing defect tracking to the apex on one surface — particularly in a tooth with an otherwise healthy periodontium — is the classic presentation of a sinus tract from an endodontic lesion, not true pocket formation. Wide, generalised pocketing across multiple teeth in a patient with established periodontitis changes the working diagnosis entirely. Treatment Sequence For primary endo lesions: treat the endo first and only the endo. Allow six to eight weeks for the tissues to respond before reassessing. In the majority of cases, the pocket resolves without any periodontal intervention. Initiating perio treatment concurrently — scaling the root surface, performing surgical debridement — in a lesion that is fundamentally endodontic will not improve the outcome and may complicate tissue healing. For combined lesions: endo first, then perio. The rationale is the same — eliminating the endodontic source of infection first allows you to assess what true periodontal disease remains once the endo component has resolved. What looks like a severe combined defect before endo treatment can look very different eight weeks later. For primary perio with secondary endo: perio is the primary treatment. Endodontic treatment in addition to perio is reasonable where there is clear pulpal involvement, but the prognosis is driven by the periodontal component, not the endodontic one. Be honest with patients about what successful endo treatment will and won't change. When to Refer Refer before you treat if any of the following are true: vitality testing is inconclusive, the CBCT shows a complex three-dimensional defect, you suspect furcation involvement alongside periapical pathology, or you are uncertain which type of lesion you're dealing with. The treatment that helps a primary endo lesion can actively harm a true combined lesion if the periodontal component is underestimated.
